In recent years there has been a significant decrease of newly infections with HIV accompanied by an increase in improved specialized health care for people living with HIV in many parts of the world. However, there are still reasons to worry, particularly in regard to certain vulnerable social groups, which find themselves at greater risk of contracting the virus, such as people who inject drugs (PWID). This paper highlights the complexity of the issue of the HIV response among PWID and presents specific challenges that arise from it. The rise of new psychoactive substances, social justice concerns, the representation of the fight against HIV in the Millennium Development Goals as well as traditional and human rights based responses to the drug problem are analyzed. In the end a personal viewpoint on what constitutes an efficient HIV response for PWID is presented.
