Evaluation of the effectiveness of a home-based social program for the elderly using logic model. The aim of this study is to evaluate the performance of a home-based social program for the elderly, focusing the attention on program outcomes, impact and effectiveness. The main approach applied to evaluate the program was the success case method using logic model. The evaluation research has used a case study design. The participants to the research have covered two populations – 36 primary clients of the program between 65 and 90 years old, and the staff who has delivered the direct home-based services for the elderly. The data collection methods have combined individual structured interviews, participant observation and organizational documents analysis. The results of the evaluation research were analyzed using descriptive statistics and content analysis of data collected. The findings show that the quality of life for all the clients served by the program have improved at least in some dimensions: 89% of the clients changed their affective state, 89% of the older people have extended their social support network and improved their interpersonal relationships, 78% of the clients improved their physical functioning, 78% of the older people improved their general state of health. Personal autonomy has increased for all the older people served by the home-based social program. The service evaluated has proved to be an effective and sustainable one and we recommend it to be scaled up in other regions, too.
